Value-Based Review

For ALL Surgical area submissions please have the physician champion submit the request. This includes but is not limited to General, Spine, Moran Eyes, Orthopedics, Neuro, and Neurovascular surgical requests.

For the following departments, follow the link below to submit requests:

Cardiac, Cath Lab, Interventional Radiology, Commodity Products, Endoscopic Procedures, Reprocessing Products and Waste.

• Send a New Product Request to productsandevaluation@hsc.utah.edu
  • This will provide a packet of information on what needs to be submitted for consideration  
• Obtain a university clinical sponsor for the new product
• Follow the instructions in the packet you’ll receive
