Home Based Care | Office of Population Health
Home-Based Care in an important part of Population Health. It helps more people get the care they need, keeps patients connected to their care provider after they leave the hospital, and improves patient outcomes. Through a partnership with Community Nursing Services (CNS), we provide care at home to patients all over Utah. CNS works with University of Utah Health to make sure patients get the same expert care at home as they would in a hospital. We focus on making recovery safe, comfortable, and effective by staying in close contact with U of U healthcare teams.
The Heal at Home program helps patients recover at home after being treated at the University of Utah Health's hospitals. Patients can keep healing in the comfort of their own home while still getting the same high-quality care they would in a hospital.
