Reporting to the Utah Cancer Registry

Cancer is designated as a reportable disease in the state of Utah (Cancer Reporting Rule, R384-100).

Laboratories that identify reportable conditions for Utah residents or ordered by providers in Utah should submit reports about those conditions regularly to the Utah Cancer Registry.  It is required to report cancer cases electronically. What to report

We prefer electronic pathology reports in HL7 format following the NAACCR Volume V standard. Pathology Reports should be selected using both Core and Expanded ICD-10-CM codes.  Each report should include the following information:

  1. Laboratory and ordering provider information
  2. Patient’s demographic information
    • Name (first, middle, last), date of birth, sex, race, ethnicity, state of residence. If available: social security number, full address, and phone number
  3. Diagnosis
  4. Results of cancer-directed laboratory examination, assays, and tests

How to Report Cancer Cases

Pathology reports must be submitted electronically. We will work with you to determine the best submission format and method for your laboratory.
