Please fill out as much information as you have available including a summary, IRB Number (if relevant) and who the Principal Investigator is for the study. If the primary contact person will be someone other than the PI, please note who should be contacted in your summary.
Choose your severity level and due date (if applicable) appropriately. If you have relevant files for the request, you may attach them.

Once you click Create, your request will be sent to UPDB to be assigned to a member of the team. Once assigned, you will be contacted to discuss the details of your request and to obtain any other relevant files or information.
