Clinical Training Agreements

Standard Pre-Approved Health Care Clinical Training Agreements (CTAs) Are Used When:

  1. Students are placed at U Facilities, or
  2. University of Utah students are placed at non-University facilities (excluding Intermountain Healthcare (IHC), which is governed by a separate agreement).

Clinical Training Agreements (Word)

  • Clinical Training Agreement (CTA) – U Students Placed at Non-U Facilities
    Used for University of Utah students completing training at external facilities.

  • Clinical Training Agreement (CTA) – Students Placed at U Facilities
    Used for external students completing training at the University of Utah facilities.

  • Governmental Entity Addendum (GEA)
    Used alongside an external site’s agreement, only if the site requires use of its own form instead of the University’s. 

    • The University of Utah is a governmental entity of the State of Utah and a public institution of higher education, and is subject to a number of laws and policies specifically applicable to these types of entities, including Utah's government records act, Utah’s governmental immunity act, and Utah’s student data protection law. Also, as a state entity, the University is insured through the State of Utah's Risk Management Office and is represented by the Attorney General's Office in certain matters. This addendum addresses these and other related issues.

For questions regarding the Clinical Training Agreement (CTA) process, please contact the AVP-HS Education Office. Before reaching out to the Office of General Counsel (OGC), departments should first confirm with the AVP-HS Education Office whether an agreement already exists with the training site.
